• Final Participant Guarantee Form
  • Use this form to send your final, guaranteed numbers to CCIS.

• Room List    Please select the room list with the occupancy type that matches your group's maximum occupancy as presented in your Pre-Trip Planning Proposal and/or Contract.

• Please make duplicate copies of the room list form should your group require more than 10 rooms.

• Hotel Information Form
  • This form helps us to better handle your lodging arrangements.
• Charter Company Information Form
  • Use this form when you are handling your own transportation arrangements.
• Performance and/or Clinic Information Form
  • This form is for our groups who are performing or attending an educational clinic, and helps us to ensure that all of your needs are met.
• Broadway Show Selection Form
  • For our groups headed to see a show, use this form to choose your selected shows.
• Group Leader Site Visit Request Form
  • As an added value benefit, we offer our client group leaders a chance to visit their destination city ahead of the group.
  • Use this form to send a request to CCIS for a site visit.
• Tour Evaluation Report Form
  • We very much value our clients input on our trips. Use this form to tell us how your trip with CCIS went.
