These forms are for the use of our current clients.

Due to the fact that these two forms require signatures, they must be faxed or mailed.

For the following forms, please select the form(s) required and download to your computer. You can then complete the form and submit to CCIS via mail, fax, or as an email attachment.

• Room List

  Please select the Room List with the Occupancy Type that matches your group's maximum occupancy as presented in your Pre-Trip Planning Proposal and/or Contract. Please make duplicate copies of the Room List form / file should your group require more than 10 rooms.
